The Practical Playbook: Helping Public Health and Primary Care Work Together to Improve Population Health

The Practical Playbook is a resource hosted on a website that provides and promotes practical tools and resources to facilitate cross-sector partnerships, training, and collaborations to improve population health in collaboration with primary care. This resource is intended for public health, primary care, and other organizations across the United States and contains a repository of existing partnerships and funding (by state) as well as tools and process to develop new partnerships. Educators may use this resource to further promote advocacy for population health and find existing examples that can be analyzed or use the many resources available to teach what is critical to a successful partnership or collaboration with community partners and primary care. —Ashti Doobay-Persaud, MD, NCEAS
