Welcome to our Community of Practice!

Our community of practice is an interprofessional group of healthcare professionals who are passionate about teaching and learning about the social determinants of health. With the goal of fostering collaboration and discovery, community members contribute in a variety of ways to advance change in preparing the next generation of healthcare professionals to address social determinants of health.
