Interdisciplinary Education Apartment Simulation (IDEAS) Project: An Interdisciplinary Simulation for Transitional Home Care

This resource is a written SIM case outlining a home visit with a patient and a multi-disciplinary care team. Learning/participation involves problem solving, observation, clinical judgement and team communication skills. Participants are expected to recognize and address the patient context for a person with physical and social barriers to health access. It is a well written and thought provoking case, but complex to put together. Set up includes finding a suitable space that can approximate a person’s abode, a well skilled SIM actor, and several pieces of medical equipment including walkers, bedside commodes and such, as well as pill bottles and smaller items, etc. Written materials are provided to guide facilitation through this case.
