Population Health & Health Equity Rotation

This resource contains the curriculum for a Population Health and Health Equity Rotation for family medicine resident. There are 4 PDFS that contain expectations, assignments and activities guidelines for the program. The main objective of the program is to understand and learn about community health as well as a range of high priority populations. The residents participate in several clinics that serve a diverse set of populations and learn about home visits, homeless shelter and paramedics services to understand a population’s specific needs and how to organize a clinical practice to serve these needs. There are interactive readings, reflections and assignments that allow for learners to assess their knowledge and understanding.
