ACO Learning Collaborative: Addressing Social Drivers of Health

This online curriculum from the Institute for Accountable Care was developed to guide health care systems in creating and implementing strategies to address the social drivers of health. The curriculum spans seven sessions covering different topic areas, from establishing SDOH screening practices to connecting patients with community resources. Each session includes links to powerpoint presentations on the various topics. There are also three accompanying issue briefs that provide additional detail. The format is primarily didactic with limited interactivity, with some poll-based questioning interspersed throughout the presentations. The presentations are thorough but do not include notes, and so implementation would require a faculty member with expertise in the topic area. The intended audience for this curriculum is at the health care systems level, making this curriculum less applicable for trainees and beginning learners as-is.
