Step Two
Complete the form below to upload your original SDOH Assessment Tool. If you are uploading multiple files, please select them one at a time. Do not upload zip files. Once tools are reviewed, our team will contact you with their decision. All accepted tools will be included in the SDOH Assessment Toolkit with full credits given to the submitting faculty and institution, and the faculty will be sent an e-gift card for $25. Tools that have previously been submitted by another faculty will not be accepted.
