Learn more about the Education-Centered Medical Home (ECMH).

The ECMH is a novel, team-based, longitudinal primary care clerkship founded on the principle of continuity and focused on teaching the educational principles of the Patient-Centered Medical Home (PCMH) delivery model. The ECMH embeds teams of medical students into existing primary care clinics where they care for an authentic panel of medically complex patients over time. The four-year structure with stable relationships amongst patients, preceptors, and peers allows its trainees to see the natural history of chronic illness firsthand and, therefore better appreciate the profound impacts of social determinants of health on patient outcomes over time.
